It is called the $100 Apple Store Credit for Early iPhone Owners. So if you bought your iPhone before August 22nd head over and fill the form by November 30th.

I’d try to spend most of it in the iTunes Plus section but since it’s not a very big catalog yet, I don’t know. I’ll probably just get something in a retail store.
